Information
- The individual or authorized representative must sign the time sheet at the end of the pay period. If you have shared supports then someone must verify for each of the individuals on the timesheet.
- The time on your timesheet and notes must match the time you clocked in and out on LaSRS.
- If you forget to clock in when you get on shift, clock in as soon as you remember on your cell phone and call the office to correct your time.
- Timesheets and notes are due in the office on designated Mondays by 10:00AM.
- A user is allowed 5 attempts to log into the site. If a user fails to login successfully, the user will be locked out of LASRS and will need to call the office to be unlocked. Please be careful when logging into the site!!
- If you get locked out of your account on the weekend or outside of business hours, please call the office on the next business day. (Same rules apply if you forget to clock in or out, have any questions or problems with the system)
- Call the front office or someone in payroll for assistance.
- Call the staffing coordinator or the front office if you are working with a new individual and they are not on your service check in/check out list.
- When writing your notes use black or blue ink only.
- When writing your notes the opening note needs to begin with Shift Begins, who you received report from, the persons first and last name (unless it was the parent) and what your individual was doing when you arrived.
- At the end of your shift, begin with Shift Ends, who you gave report to, first and last name of the person (unless it was the parent) and what your individual was doing when you left.
- After your end of shift note, you must go to the next line then draw a line, then sign your first and last name with DSW after it. When you go back for your next shift, skip one line between shifts.
- When bathing an individual, you must always check the temperature of the water to make sure it isn’t scalding and write that you checked it in your note.
- You need to use the entire front and back of your note sheet before you start a new one.
