PERSONAL APPEARANCE:
- Always be clean, neat and free of body odor. No excessive perfume.
- Fingernails must be kept short.
- Never wear clothing that is too tight or revealing. Regular clothing that allows you to blend in with the participant is preferred. If you work for a participant that is medically needy then scrubs may be worn.
- Slippers, open toed shoes, flip flops or jogging suits are not allowed.
COURTESY:
- Do not make phone calls from a participant’s home. You may call the office, if necessary, after obtaining permission from the participant or family.
- No smoking on the job.
- All phone calls go through the agency. Do not give your phone number to the participant. Never call the participant at home.
- Do not give the participant’s phone number to anyone. If the family needs to contact you in an emergency, they must call the office and we will contact you.
- Provide your own meals and snacks.
- Avoid using cell phones, tablets or e-mailing on the job.
- DO NOT USE CELL PHONE WHILE DRIVING.
CONFIDENTIALITY:
- Do not talk about a participant or his family to anyone outside of this office.
- Your friends or family members are not allowed at the participant’s home.
- You are NEVER allowed to bring a participant to your home.
CANCELLATION POLICY:
- You can contact an on-call manager 24 hours a day, 7 days per week at: 337-233-0545 or 1-800-749-0545. Use only these numbers to reach someone after hours.
- If you are unable to work, the office is to be notified IMMEDIATELY.
- Call in for yourself; do not have family or friends call in for you.
- If you are running late call us. Do not call the participant at home.
TIME SHEET & NOTES POLICY:
- Time sheets and notes must be in on designated Monday’s, no later than 10:00 a.m.
- Checks are available to be picked up between 8:00 a.m.–4:00 p.m. on payday Friday’s. If you have not picked up your check by 4:00 p.m., it will be mailed to you.
- You must notify us in advance in order to have someone else pick up your check. The person must have valid identification.
- Always chart in black or blue ink only.
- NEVER complete time sheet or note prior to working.
- You can only drop off notes & time sheets in the drop box when the office is closed, and they must be in an envelope.
- TIME SHEETS AND NOTES MUST STAY IN THE PARTICIPANT’S BINDER UNTIL IT IS TIME TO TURN THEM IN TO THE OFFICE ON DESIGNATED MONDAY’S.
REQUESTS FOR TIME OFF
YOU MUST TURN IN REQUESTS FOR TIME OFF TO THE STAFFING COORDINATOR IN WRITING AT LEAST 2 WEEKS IN ADVANCE TO BE CONSIDERED FOR TIME OFF
MISCELLANEOUS:
- Do not discuss your pay with anyone, including other workers and participants. Do not talk about needing a raise or your schedule.
- You must give us a two-week notice and honor it if you plan to resign from the company and want to be eligible for rehire or receive a favorable work reference.
- IT IS MANDATORY that all Critical Incidents are reported to case manager or our nurse IMMEDIATELY.
- No personal business should be performed while on the clock.
- You must arrive at work 10 minutes before the start of your shift.
- YOU CAN NEVER LEAVE A PARTICIPANT ALONE.
- YOU CAN NOT WORK MORE THE 40 HOURS A WEEK WITHOUT APPROVAL FROM THE OFFICE.
